How to Inject Ozempic or Mounjaro: A Step-by-Step Guide for First-Timers

Your medication has arrived. You're holding the pen. Now what? For most first-time injectable medication users, this moment feels more intimidating than it actually is. The Ozempic and Mounjaro pens are among the most patient-friendly drug delivery devices available — and within a few weeks, the weekly injection becomes as routine as taking a morning supplement. This guide walks you through everything you need to know.

Before your first injection: Let the pen sit at room temperature for 30 minutes. Cold medication causes more discomfort. Check the expiry date. Inspect the pen window — the solution should be clear and colourless (for Ozempic) or colourless to slightly yellowish (for Mounjaro). If it looks cloudy, has particles, or has changed colour, do not use it — contact your pharmacist.

The Three Approved Injection Sites

Abdomen
5 cm away from the belly button, anywhere around the circumference. Most popular site — easiest to access and self-administer.
Thigh
Upper outer thigh — the front-outer quadrant. Easy to access when seated. Avoid the inner thigh and knee area.
Upper Arm
Outer upper arm — the soft, fatty area. Slightly harder to self-inject but excellent for rotation. Not the shoulder or inner arm.

Rotate sites each week — don't use the exact same spot more than once every 4–6 weeks. Repeated injection in one location causes lipohypertrophy (hardened, lumpy fatty tissue) that reduces medication absorption and can cause inconsistent results.

Step-by-Step: Your First Injection (Ozempic Pen)

Wash your hands thoroughly

With soap and water for at least 20 seconds. Dry completely. Clean hands are your primary infection prevention step — you don't need to swab the injection site with alcohol unless your skin is visibly dirty.

Check and prepare the pen

Remove the pen cap. Check that the dose indicator window shows your prescribed dose (0.25 mg, 0.5 mg, 1.0 mg, or 2.0 mg). For a new pen: turn the dose selector to the flow check symbol (two dots) and perform a flow check — press the button and verify a small drop appears at the needle tip.

Attach the needle

Remove a new needle from its packaging. Remove the paper tab. Screw the needle onto the pen (clockwise, firmly but not over-tightened). Pull off the outer needle cap and keep it for disposal. Pull off the inner needle cap and discard it.

Set your dose

Turn the dose selector until it shows your prescribed dose in the dose window. You should hear and feel a click for each dose increment. Do not push the dose button while setting — this will eject medication. Check that the correct dose appears in the window before proceeding.

Choose your site and prepare the skin

Select your injection site (rotating from last week). Gently pinch a fold of skin with your non-dominant hand — this is especially helpful for patients with lower body fat. Wipe with an alcohol swab only if the area is visibly soiled; otherwise this is optional. Wait for skin to dry completely if you do swab.

Insert the needle

Hold the pen like a pencil — firmly but not white-knuckle tight. Insert the needle at a 90° angle (straight in) in one smooth, confident motion. Hesitating or going slowly increases discomfort. The needle is 4–6 mm; it does not go deep. If you're very lean, you can inject at a 45° angle to avoid muscle.

Press the button and hold

Press the dose button slowly and completely until it reaches zero. Hold the button pressed and count to 6 slowly before withdrawing the needle. This is the most commonly skipped step — withdrawing too early causes medication to leak out rather than absorb. You should see and hear the dose counter reach zero.

Withdraw and dispose safely

Withdraw the needle in the same angle it entered — smooth, straight out. Do not rub the injection site (this can cause bruising). Replace the outer needle cap immediately using the cap on a flat surface (never recap by holding the cap). Remove and dispose of the needle in a sharps container or hard-sided container. Replace the pen cap for storage.
