Am I Eligible for GLP-1 Weight Loss Medication in South Africa?
By Metabolic Doc · Updated April 2026 · 7 min read
Before you book a consultation or start researching GLP-1 medications like Ozempic or Mounjaro, the most important question to answer is: do I actually qualify? This guide explains the clinical criteria used to assess eligibility, what your doctor will look for during the consultation, and what happens if you don't meet the standard threshold.
Important: The criteria below are general guidelines. Ultimately, it is your doctor's clinical assessment — not a checklist — that determines whether GLP-1 therapy is appropriate for you. If you're unsure, the right place to find out is during a consultation.
The Primary Eligibility Criteria
GLP-1 therapy in South Africa follows internationally recognised clinical guidelines. The standard criteria are based on BMI (Body Mass Index) and the presence of weight-related health conditions:
| BMI Range | Classification | Eligible for GLP-1? |
| Below 25 | Healthy weight | Generally no |
| 25 – 26.9 | Overweight (borderline) | Generally no |
| 27 – 29.9 | Overweight | Yes — if weight-related comorbidity present |
| 30 and above | Obesity | Yes |
How to calculate your BMI
BMI = weight (kg) ÷ height (m)². For example, if you weigh 95 kg and are 1.70 m tall: 95 ÷ (1.70 × 1.70) = 95 ÷ 2.89 = BMI of 32.9 — well within the qualifying range.
Note on BMI limitations: BMI is a screening tool, not a perfect measure of health. People with significant muscle mass may have a high BMI without being overweight. Conversely, some patients with a BMI under 30 carry significant metabolic risk. Your doctor takes the full clinical picture into account, not BMI alone.
Weight-Related Conditions That May Lower the BMI Threshold
If your BMI is between 27 and 29.9, you may still be eligible for GLP-1 therapy if you have at least one of the following weight-related health conditions:
- Type 2 diabetes or prediabetes
- High blood pressure (hypertension)
- High cholesterol or triglycerides (dyslipidaemia)
- Obstructive sleep apnoea
- Non-alcoholic fatty liver disease (NAFLD/NASH)
- Polycystic ovary syndrome (PCOS)
- Osteoarthritis worsened by excess weight
- Cardiovascular disease or significant cardiovascular risk
Who Is and Isn't Eligible: Side-by-Side
✓ Likely Eligible
- BMI ≥ 30 (any reason)
- BMI 27–29.9 + diabetes, hypertension, PCOS, or dyslipidaemia
- History of failed diet/exercise attempts
- No relevant contraindications
- Not currently pregnant or breastfeeding
- Non-diabetic patients wanting weight management
✗ Not Eligible
- BMI below 27
- Personal or family history of medullary thyroid carcinoma (MTC)
- Multiple Endocrine Neoplasia type 2 (MEN2)
- History of pancreatitis
- Pregnant or breastfeeding
- Severe gastroparesis or GI motility disorder
- Severe kidney impairment (some medications)
- Type 1 diabetes (GLP-1 not first-line)
What Your Doctor Will Assess During the Consultation
The purpose of the consultation is to make a thorough, individualised assessment — not just to tick boxes. Your HPCSA-registered doctor will review:
- Your BMI and current weight — calculated from the height and weight you provide
- Medical history — including all current diagnoses and previous health issues
- Current medications — to check for interactions (especially insulin and other antidiabetic drugs)
- Family history — particularly thyroid cancer, MEN2, or pancreatitis
- Previous weight loss attempts — diet programmes, medications, bariatric procedures
- Weight-related comorbidities — which may affect both eligibility and medication choice
- Your goals and expectations — to ensure GLP-1 therapy is appropriate for your situation
Do I Need Blood Tests First?
Recent blood tests are helpful but are not always required for an initial consultation. If you have recent results (within the past 6–12 months), it's worth having them available. Your doctor may specifically recommend tests before or after prescribing, particularly if:
- You have type 2 diabetes (HbA1c helpful)
- You have kidney disease concerns (renal function important)
- You have liver disease concerns
- Your doctor wants to rule out secondary causes of weight gain (e.g., thyroid function)
If you don't have recent bloods, don't let that stop you from booking a consultation. The doctor will advise you on what, if anything, is needed in your specific case.
What If I Don't Quite Meet the Criteria?
If your BMI is just below 30 and you don't have a formal weight-related comorbidity, you may not qualify for GLP-1 therapy under standard guidelines. In this case, your doctor will:
- Explain why GLP-1 therapy may not be clinically appropriate at this time
- Suggest alternative evidence-based approaches (dietary counselling, exercise programmes, other medications where appropriate)
- Advise on a plan that may make you eligible in future if your situation changes
The doctor's clinical judgement is always final. No reputable medical practice prescribes GLP-1 medications to patients who don't meet the eligibility criteria — doing so would be clinically irresponsible and ethically problematic.
Frequently Asked Questions
What BMI do I need for Ozempic or Mounjaro in South Africa?
The standard threshold is a BMI of 30 or above. Patients with a BMI of 27 or above may also qualify if they have a weight-related health condition such as type 2 diabetes, hypertension, dyslipidaemia, sleep apnoea, or PCOS.
Can I get GLP-1 medication if I don't have diabetes?
Yes. You do not need to have diabetes to qualify for GLP-1 weight loss therapy. Obesity (BMI ≥30) alone is sufficient. Diabetes is one of several comorbidities that can lower the BMI threshold to 27.
Who should NOT take Ozempic or Mounjaro?
GLP-1 therapy is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC), MEN2 syndrome, a history of pancreatitis, severe GI disease, or during pregnancy and breastfeeding. Your doctor will assess all contraindications during the consultation.
Do I need to have tried other diets first?
There is no formal requirement to have completed a specific diet programme before being eligible. However, GLP-1 therapy is generally considered for patients who have not achieved adequate results through lifestyle measures alone — which describes the vast majority of people with obesity.
My BMI is 28 and I don't have any diagnosed conditions — can I still qualify?
At a BMI of 28 without a formally diagnosed comorbidity, you are at the borderline of the standard eligibility criteria. Whether you qualify depends on your full clinical picture, including undiagnosed conditions and metabolic risk factors. The doctor will assess this during the consultation — it costs nothing to find out.
